The Montgomery County Health Department provides various services aiming to prevent disease, control its spread, and promote public health awareness. These services are available to individuals of all ages who meet eligibility requirements. Services are provided to the insured, underinsured, and uninsured. Most services are offered on a sliding fee scale based on income and family size.

Women, Infants, and Children (WIC)
WIC is a nutrition program for Women, Infants, and Children. Eligible participants include pregnant, postpartum, and breastfeeding women and infants and children up to 5 years old who meet guidelines. Individuals receiving food stamps, AFDC, or TennCare are automatically income-eligible for WIC. Contact: (931) 551-8777 Fax (931) 503-0691
When you come in for your certification visit and when you recertify, make sure to bring these items with you.
- Proof of Identity (driver’s license, social security card, shot record, or birth certificate)
- Proof of Residency (utility bill/receipt, rent/mortgage receipt, or a piece of mail with your name and current address)
- Proof of Income for the Household (paycheck stubs, SNAP eligibility letter, TANF eligibility letter, or a bank statement)
Immunizations and Back-to-School Shots
Vaccinations are an effective way of protecting people against harmful diseases. In the last 10 years, several pharmacies have joined the Back-To-School Immunization Partners. Along with the health department, many of our partners provide immunizations against polio, diphtheria, whooping cough, lockjaw, measles, mumps, rubella (German measles), Hemophilus, and Hepatitis B, Influenza (flu), and pneumonia immunizations as well as diphtheria-tetanus boosters are available to adults for lasting protection against these diseases.
Primary Care and Telehealth Services
Primary care services include diagnosing and managing acute and chronic illnesses, wellness screenings, immunizations, education, prevention, and laboratory testing. Assistance with medications and supplies for treating common conditions may be available to those who qualify. Telehealth appointments are available to patients to improve access and quality healthcare for patients.
Well Child Exams
Well-child services for individuals from birth to 21 years of age include physical exams, screening tests, and immunizations. This is to ensure normal development and to detect and treat various medical conditions.
Diabetes Team and Nutrition Services
A registered dietitian is available to provide medical nutrition therapy for those with special needs such as diabetes, hypertension, or weight management.
The diabetes team facilitates a CDC-recognized Diabetes Prevention Program twice a year which is free of charge and open to all community members who have prediabetes or are at risk for diabetes. The year-long program teaches participants to make lasting lifestyle changes—like eating healthier, adding physical activity into their daily routine, and improving coping skills. Community Health Access and Navigation in Tennessee (CHANT)
CHANT is an integrated model of care coordination. The program provides enhanced patient-centered engagement, assistance navigating complex systems, and care coordination of medical and social service needs. Families and individuals are connected to available resources, which may include reimbursement for medical services and assistance with co-pays, deductibles, and co-insurance for children and youth with physical disabilities.
Children's Special Services (CSS)
This service assists with medical treatment for children to age 21 when they have special medical needs, and the family is financially unable to provide the necessary care. This service may also include speech and hearing screenings.
Dental Clinic
Dental Services are available to uninsured children and adults and those with TennCare

Family Planning
Family Planning services include counseling, pelvic and breast exams, pregnancy testing, contraceptive supplies, and sexually transmitted infection counseling, testing, and treatment.
Prenatal Services
Prenatal services include pregnancy testing, assistance with applying for temporary TennCare coverage, counseling, and referral to a physician for prenatal care.
Breast & Cervical Cancer Screenings
Breast and Cervical Cancer Screening Services are available to womenaged 18 and older who are uninsured or underinsured and meet the program's income and screening guidelines. These services include clinical breast exams, pap smears, and mammography for thoseat risk of breast and/or cervical cancer.
Birth and Death Certificates
Certified copies of birth certificates for individuals born in Tennessee, as well as death certificates, can be purchased. Paternity acknowledgements for birth certificates are also provided. Communicable Disease Services
The health department provides FREE and confidential testing and treatment for syphilis, gonorrhea, chlamydia, and HIV. Contact follow-up conducted to avoid further spread of disease. Services are available without parental consent. Testing is done on a walk-in basis until clinic capacity is met for the day. You may call to make sure the walk-ins are available before arriving.

Pregnancy Tobacco Cessation
Tennessee’s Pregnancy Tobacco Cessation Program is a program created to improve the health and wellness of women and families across Tennessee by providing them with resources to quit smoking. This program combines evidence-based practices with support and offers hope for parents and babies in Tennessee that want to be tobacco free.
Overdose Risk Reduction
Naloxone is used as a reversal in an opioid overdose. Limited supplies of naloxone are available free of charge.
Health Educators
Our health educators are here to offer free educational information and programs for schools, churches, workplaces, community groups, and individuals. Health topics include nutrition and healthy eating, sexual health, tobacco awareness and prevention, health risks of physical inactivity, employee wellness, substance abuse, cyberbullying, internet safety, and overall health and wellness. They also offer free smoking cessation classes.
Motor Voter Program
Is available and assistance in completing the forms will be provided.
